The
Children’s Health Improvement Partnership
(CHIP) of New River Valley continually builds and
maintains a network of local public/private partnerships
to improve the health status of young children in need and
to support and enhance their family’s ability to move
toward self sufficiency. CHIP builds on family strengths
and promotes family empowerment.
The
purpose of CHIP is to assist low-income families with children
up to age 7 and pregnant women with access to medical and
dental services and act as a link to other human service
organizations/providers in the NRV. The CHIP model is unique
to the New River Valley in that it offers comprehensive
case management in the home where children receive consistent
and comprehensive care in a familiar setting. CHIP services,
provided by a nurse and a home visitor team, include helping
families make and keep appointments and follow physician
recommendations, educational home visits, parenting education,
helping families set and reach personal and family goals,
transportation to medical and dental appointments, medical
case management, and referrals to other community-based
services to help meet the multifaceted needs of families.
All services emphasize prevention and early intervention.
CHIP
of New River Valley is funded by CHIP of Virginia, the United
Way/Fund, the United Fund of Giles County, the Virginia
Department of Social Services-Promoting Safe and Stable
Families, foundations, private donations and other local,
state and federal funds.
